My client is looking for a Social Worker to join thier team for at least 3 months. You would be working within the community care sector assisting the unit providing a responsive and needs led service.

Your main duties would include -
Draw up, monitor and review care plans
Negotiate with service providers and cost packages of care
Liaise with colleagues from other agenicies
Support, Counsel and work proactively with service users.

For this post you must be fully qualified and registered with at least one years previous experience working within the Community Care field.

Due to the nature of your work you will be out visiting people at home so you must be able to drive and have your own car.
